It was working fine until a raccoon ran out in front of me and I hit him. It doesn't read anything but the "0C" now.
Thank you so much to the people that provide an answer! I truly appreciate it! The answers are great.

It will be mounted behind the grille, but in front of the radiator. It is a round sensor with a square plastic bracket held in place by one screw.

You might have to remove the plastic cover that goes between the grille and the radiator support to see it and get to it.

The sensor is located in the grill. However the 'OC' means 'open circuit'. Raise the van on a lift and look under it where the raccoon went under and see if you can find some wires damaged or pulled loose.
